What is the VA Community Care Network?
The Department of Veterans Affairs (VA) understands that sometimes the best place to receive care is right in your own community. The VA Community Care Network (CCN) is a program that allows eligible veterans to receive home care services from approved local agencies—like Dream Living Support Services—when the VA cannot provide those services directly.
Our mission is to help veterans in Georgia age in place safely and comfortably. We handle the coordination, provide highly trained caregivers, and bill the VA directly, so you and your family can focus on what matters most: your health, dignity, and peace of mind.

Eligibility & The Referral Process
Navigating VA benefits can feel overwhelming. Here is a step-by-step guide to how the Community Care Network referral process works.
Consult Your PCP
Schedule an appointment with your VA Primary Care Provider (PCP). Discuss your daily struggles and request a clinical evaluation for home care services.
VA Authorization
If your PCP determines you have a clinical need, they will submit a referral. The VA will review your eligibility and authorize a specific number of weekly care hours.
Provider Selection
You have the right to request a specific agency. Tell the VA you want Dream Living Support Services as your local community provider.
Care Begins
Once we receive the authorization from the VA, our care coordinator will meet with you to design a personalized schedule and match you with a dedicated caregiver.
