Dream Living Support Services provides structured, non-medical in-home care designed to support safety, independence, and quality of life for seniors and adults throughout Metro Atlanta and surrounding counties.
We coordinate with hospitals, rehabilitation centers, case managers, and families to ensure safe transitions home and continuity of care.
All services are provided in full compliance with Georgia Private Home Care Provider (PHCP) regulations. We do not provide skilled nursing, medication administration, injections, wound care, therapy, or medical treatment.
We work alongside discharge planners and care teams to support safe transitions from hospital or rehabilitation settings into the home environment.
We assist families with understanding eligibility, navigating authorization processes, and coordinating required documentation when applicable.
We begin every client relationship with a structured, no-obligation in-home assessment to determine support needs and develop a personalized care plan.
